Case Manager

Position Summary
The Case Manager is responsible for coordinating and delivering individualized services to youth in the residential program. This role ensures that each resident receives comprehensive care through assessment, intake, service planning, advocacy, and coordination of community resources. The Case Manager also provides mentorship and support, serving as a primary link between residents, families, service providers, and the agency to promote healing, growth, and successful outcomes.
Key Responsibilities

Conduct intakes and complete all required admission paperwork in accordance with program standards and regulatory requirements.

Develop individualized service plans in collaboration with residents, families, and the treatment team.

Provide case management services addressing mental health, education, substance use, health care, and family support needs.

Monitor residents’ progress and update service plans regularly to reflect changing needs and goals.

Facilitate family involvement and support family reunification efforts when appropriate.

Teach and reinforce life skills, including communication, decision-making, problem-solving, and self-management.

Coordinate access to counseling, educational support, health and wellness services, financial literacy, and recreational opportunities.

Provide transportation as needed for residents to doctor’s appointments, court hearings, school, family visits, and other approved activities.

Serve as an advocate for residents, connecting them to community resources and ensuring their voices are included in decision-making.

Maintain compliance with Pennsylvania Chapter 3800 regulations and all agency policies and procedures.

Document case notes, progress reports, and required records in a timely and accurate manner.

Participate in team meetings, staff trainings, and professional development to maintain high standards of care.
Qualifications

Associate’s degree in Social Work, Criminal Justice, Psychology, Counseling, or a related human services field (Bachelor’s preferred).

Minimum of 1–2 years’ experience in case management, counseling, or human services (experience with youth and/or residential settings strongly preferred).

Valid driver’s license with clean driving record; ability to transport youth safely in agency vehicles when needed.

Knowledge of trauma-informed care, behavioral health, and evidence-based practices.

Strong communication, organizational, and problem-solving skills.
